To request a copy of your medical records, please follow these instructions to request a copy of your records from the Howard University Faculty Practice Plan

  1. Download and complete the Authorization to Disclose PHI form.
  2. Return form via email, fax, directly to the medical records office or at your next office visit.
    1. Email requests to MedicalRecordsRequest@Howard.edu
    2. Fax requests to 202-865-3261
    3. Address: Howard University Hospital, Towers Building, 6th floor, 2041 Georgia Avenue, NW, Washington, D.C. 20060
  3. For non-urgent requests, we will process request within 30 days. For urgent requests, we can process within 72 hours.

Pharmacy

Visitors to the Faculty Practice Plans have access to the Walgreens Pharmacy for their prescriptions, which is conveniently located on the first floor of Howard University Hospital.

The Pharmacy hours are:

  • Monday - Friday 9:00 a.m. - 6:00 p.m.
  • Saturday: 10:00 a.m. - 5:00 p.m.
  • Sunday: Closed

Public Transportation

The Faculty Practice Plan is serviced by the Shaw/Howard University Station on the WMATA Yellow and Green Lines. In addition to four MetroBus routes (70, 79, & 90, 92). Patient Financial Services

The Department of Patient Financial Services is responsible for assisting patients with matters concerning Howard University Faculty Practice Plan Accounts. Contact Information: Howard University Faculty Practice Plan Patient Financial Services office hours are Monday through Friday from 9:00 a.m. - 5:00 p.m.

Financial Assistance Plans and Programs

Patients who cannot afford to pay for medically necessary care may apply for financial assistance from Howard University Hospital under our Financial Assistance Policy. We encourage patients needing financial assistance to contact the Financial Counseling Office at 202-865-2130.

Parking Information

Parking is available in front of the hospital and outside of the ambulatory care center (Towers Building). The paid parking lots are open from 6:30 a.m.–11:30 p.m. daily. Upon entering the lots, please remember to keep your parking ticket with you. There is a parking kiosk inside of Howard University Hospital. You will need the ticket to pay for parking before you leave.

Cafeteria

Faculty Practice Plan patients have access to the Howard University Hospital cafeteria located on the second floor. The cafeteria is a full-service eatery, serving breakfast, lunch, dinner and a bevy of snacks. The cafeteria is open from 7:00 a.m. to 10:00 a.m., 11:30 a.m. to 3:00 p.m., and 5:00 p.m. to 7:00 p.m., daily. Snacks and drinks can be purchased 24 hours a day from onsite vending machines.
